Changing the folder rescans the whole library. The track list updates within seconds, but for a big folder the first pass takes a while — audio formats browsers can't play get converted, and every new track is analyzed for bpm/key and tagged in the background. Progress shows next to the track count on the phone. Nothing in your music folder is ever modified.
